WHITEN BLEACHING TREATMENT PROTOCOL This protocol recommends a course of steps for professional bleaching. Depending on the patient and the bleaching treatment, steps may be required in a different order. 1. Patient’s anamnesis Evaluate the origin of tooth staining and check for restorations that could affect the final result (use RX if needed). Consider amending your periodical anamnesis by questioning re patient's satisfaction with oral esthetics. DO NOT PROCEED IF THE PATIENT: • • • • • • Is pregnant or breast-feeding Is in treatment with antibiotics Is less than 12 years old Has large / quality restorations in the anterior region Has implants in the anterior region Shows sensitivity to heat/cold 2. Oral cavity exam Examinate the patient’s mouth, with an accurate evaluation of soft tissues and the teeth (there should be no microleakage under restorations, fractured teeth, interproximal caries). 3. Patient’s expectations Evaluate the patient’s expectations, motivations, and attitude. Then show possibilities, limits, and contraindications on a general level and specific to the patient’s case. 4. The initial color Identify the initial color with the aid of a Vita Shade Guide, rearranged as shown. Take a picture of the teeth after the hygiene treatment and before the bleaching, or identify color with patient's active participation (with mirror in his hand) and document it. Patient’s consent Have the patient sign an agreement of consent to bleaching treatment and cost involved, or reach other form of consent as appropriate. 6. Hygiene treatment Proceed to the hygiene treatment with polishing paste in order to remove all exogenous pigments (plaque inhibits or reduces bleaching agents efficacy). 7. Choice of bleaching product. On the basis of the initial anamnesis and patient's expectations, recommend one or two bleaching methods and reach consent with patient. Opalescence products may be used successively to suit best the doctor´s and the patient´s expectations. Follow the instructions for use. If either Opalescence PF or Opalescence Treswhite Supreme takehome systems are chosen, schedule periodical recalls/check-ups. If Opalescence Boost is chosen, recommend some follow-up treatment with Opalescence Follow Up kits. 8. Recommendations for the patient Avoid or limit, during the period of the treatment, substances that could stain the teeth, or acidic substances, such as: coffee, tea, cola, red wine, fruit juice, red fruits. Avoid smoking. It is recommended to tell the patient to avoid such drinks and foods for at least one week after treatment. 9. The final color Identify the final color using the Vita Shade Guide (rearranged as at #4).Take a picture of the teeth at the end of the treatment. A definitive color change can be recorded only a few days after the end of the treatment, when the teeth are re-hydrated and there is no peroxide left. 10. Fluoride treatment At the end of the bleaching procedure, many dentists recommend a fluoride treatment. Avoid varnish applications (because the teeth should have the ability to rehydrate). Use 2 or 3 Flor-Opal preloaded trays, or use Flor-Opal in syringes, if the patient has custom trays. 5
